As someone who watches and is subscribed to a plethora youtube channels that discuss health and fitness, as well as being active myself, I have seen more than a handful of videos where individuals who have an unhealthy obsession with obese people crop up. They're surprisingly common, especially in the U.S which has a high obesity rate.

You are attempting to apply re-world and game logic which is going to always result in confusion.

You can apply that sort of logic towards the Witch who is an anorexic female who is hovering around 5-8% body fat which is stage weight for male bodybuilders and extremely unhealthy for women who require 3-5% percentage of body fat more than males to be healthy. In addition, the lower one's body fat percentage their sex drive also drops and when men and women are at competitive body fat percentage for bodybuilding (read: miserable) they have ZERO sex drive.

So in the case of the Witch even if she were sane and could understand the survivors, she would be insufferably cranky and not even the smoothest man or women when it comes to seducing would be able to get into her undies due to her being borderline a skeleton.
