What you're essentially asking, is why is Luke Skywalker dead in episode VIII but alive in episode IV.

Death toll is third (i wouldnt say last stand also counts on this, bc its an optional campaign to death toll as well)
Dead Air as fourth
Blood Harvest as fifth
Dam it is supposedly after Blood Harvest but is not the official campaign yet
then Sacrifice as the last campaign
I'd suggest reading the comic.
That's why it's called "The Sacrifice"... Bill sacrifices himself.
However, of course, video games being video games, the campaign is different.
Anyone can make the "sacrifice" in the official campaign.
Sacrifice would be the last campaign for the original survivors.
"The Passing" is the crossover one with the L4D2 crew and we see Bill's corpse.
Louis and Francis, if I remember correctly, have random lines mentioning the loss.
Also to clarify, "Dam It" is only as official as it was from Turtle Rock Studios.

I don't see them going after the "complete" or "remastered" versions.

Crash Course
Death Toll
Dead Air
Dam It
Blood Harvest
The Sacrifice
Last Stand was originally a survival only map, not quite cannon.
So anyone could make up a theory about where it'd go.
Perhaps an alternate route within Death Toll for all we'd know.
I think the game had sneaky Easter eggs hidden in it about that.
At least for the original crew.
Passing and learning about Bill would be after Sacrifice.
It belonged to the previous developers, Turtle Rock Studios.
They'd have to get permission, as well as make their own completed version.
