What causes a horde to appear?
Playing versus and one of the teams has way less hordes... Why?

Depends. The AI Director spawns them at panic events, trigger events (car alarms, etc.) or when a player gets vomited on by a boomer. It just works out better for one team sometimes, it's just how it is.

Normally when I play last map of Dead center there's always a horde even without boomer.

Played against this team in versus and there was barely any horde....

Are they cheating?

Each team always have the same horde limits that by default they are 30 common infected. But everything is depending on how they go.

For example:

Team A tend to be close together and move slowly and carefully, preventing the director from sending more hordes every 1.30 minutes. While the B team are to move all together style speed run or rush and dodging the horde, leaving them behind and thus have the free space the way to move faster and have fewer hordes.

Well now what may happen is that there is an admin that abuses and commits a trap that makes putting less hordes killing them fast, kicked or reduce the limit. On the other hand, for his opponent, he increases more hordes or calls him, causing him to never finish.

Are there any specific things that triggers a horde to spawn? Like noise? (not killing zombies in time or something...)

Simple answer: Director.

Director will spawn the horde when;

- You triggered cresendo event (open alarm door) and finale
- You get boomed by boomer
- You throw the bile bomb, it will occasionally spawn additional infected
- You didn't fight against any infected for some amount of time, director will randomly spawn the small horde

Like Maur0 said about team B. That pretty much sums up the answer to your question. The more commons you kill, the more that spawn in your path. The less you kill and keep running, the more left to have to chase you from behind. The director is limited to 30 commons at a time and can not remove zombies that are within a certain range / view of the players. Just off the top of my mind I believe the distance is 1,500 before commons / special bots are kicked and respawned infront of the survivor team.

No, the game is sometimes uneven with horde distribution. I’ve played vs games where one team barely has any CI and the other has nonstop hordes within the same chapter.

Competitive vs servers have a plugin that makes horde sizes even for both teams during events. So it goes to show you that it’s actually an issue.

@Dustin I think you ignored my post that told you why horde sizes are uneven. Not even a horde equalizer plugin can fix that.

Your horde equalizer doesn't even do what you are describing. Right from the code itself it says

"Make certain event hordes finite"

Commons will never spawn past the limit of 30. If the existing ones have not been killed, more will not spawn infront until the survivor team has exited the common existance distance.

There are two types of horde, event hordes from car alarms, map events and boomers and ambient hordes, which are entirely based on time spent and distance covered in the map (slower teams have more ambient hordes).

During a gauntlet style finale like Dead Centre you will see more hordes the slower you are due to time spent. Also where the hordes spawn specifically on maps like Dead Center 4 can make a large difference in how much commons effect the teams.
