This is Moe Novel we're talking about. We're more likely to see Half-life 3 than Moe Novel not censor something.

Japanese version isn't Moe Novel though. Pulltop has always been willing to release 18+ versions in Japan. It's when they are handed to Moe Novel that the content gets cut even on the non-Steam versions.

As the person above has said, it comes down to the publisher. In this case MoeNovel, there was no Hentai patch before since steam rules about nsfw content. rules have changed but they have to check with steam if they could realease the patch and do they even release a patch. Or they'd rather sell the 18+ patch on denpasoft for 10-20$

We already have Half Life Alyx and 2020 is a crazy year.
