Chapter 5 achievements? Epilogue and Escaping Death

Anyone has an ideia on how to get these two?
Epilogue and Scaping Death?
I had a hunch the last one is to finish the chapter without getting a single game over, but I'm too incompetent for that right now haha
Anyone who has finished the game without a game over can confirm?

Escaping Death: Just complete the mission of C4 with Ash without any failed, and complete the minigames without any failed.

No clue about Epilogue?

I've gotten escaping death (and all the prior achievements) , and 86% of the game. I still cant trigger epilogue. I'm gonna try only talking to the light doors in void house see if that changes anything.

You gotta get 100% percent on all the game except the final chapter however it seems you've already got that
