Character notes achievement issue?

So I just finished another playthrough and I now have all the character notes and didn't get the achievement. Lambert was the last one that I missed, but it didn't pop after I talked with him. I played all the way through the end and even after credits there was no pop. I tried reloading and trying again, but still didn't work.

Is there a certain point I need to have the notes filled out by? I read somewhere that people had to play again all the way through chapter 6 before the achievement registered, but I didn't trust that because on my PS5, I was able to get a missing not midway through the game and the achievement popped then on NG+. Not sure why it didn't pop on PC. Does anyone else have this issue?

EDIT: I have made it to chapter 5 and got the medal against from Beatrix for character notes. Still nothing on the achievements. So unsure what the problem still is.
