A crew member needs to be tired, hungry, freezing. You notice that he is infected when he looks glitchy and blurred.
To achieve "Patient Zero" you need to leave the planet having the whole crew (5 people) with at least one infected member.

My crew are glitchy few times in the last game. 3 times I finish this game, 2 of them with all 5 members. And every time I see 1 cutsceen where she said "At last I'm going home" But I have the achieve only for clean crew!!! (Sunday trip) What's I'm doing wrong? =(

I just got this achievement. You definitely don’t need all crew members. I got it with only one survivor. I did make sure he “felt bad” before he left but I also ate one of the crew members just in case that was what infected them. Since you don’t need everyone to leave couldn’t hurt to eat somebody in addition to the tired/freezing/hungry option.

I Have 100% achievement progress on Symmetry myself, basically i got this simply and not only by watching TheZeroEquals Gaming Channel for more context as a guide on how to infect a crew member, you do not need to kill any of your 5 crew members to pull this off, basically the AI will start to continuously slam you with attacks but the main thing you wanna keep an eye on is the AI's glitches that effect the kitchen, towards the end when you have 50 scrap for the left engine for the rocket meaning, i recommend the order you go in as: 90, 70, 50: anyways you want one of your most effective food making crewmates to camp the kitchen when you get around to the part where you build the left engine with 50 scrap to leave, the person in the kitchen will be interacting with the food console when it glitches out, therefore infecting that crew member, you can easily tell they're infected with the AI's bug after this effect, then when you rebuild the left engine for 50 scrap you should get patient zero, the devs never made a 2nd ending besides the bad ending where all your crew die, there's only 2 endings, the one with the rocket and the one where all your crew die. Good luck if you haven't gotten the patient zero achievement already for me it wasn't that difficult.
