it is a little of both. you can suspect hacking, but realistically it is conceivably both scenarios taking place.
how should people know who did the insta kill, if the fight just ends? even if it occurs if someone hits the mob. there are a lot of things decided player side, and non server side. that is where people abuse the game. putting everything server side costs a lot of bandwith and cpu power. so the best to protect the game would be some anti cheat software.
well, since the cheaters getting banned now:

Regardless of whether someone is using a 3rd party tool to create an advantage or finding a "glitch/bug" or workaround inside the existing system doesn't really matter. It still is cheating by bypassing the way the game is meant to be played and not to mention probably has some affect on the monetary stream coming in.
after kill a boss fight rollback one turn, boss dissapear and we continue fight with empty space. looking of rain of stamps it was not only a my desync
super lame, but i would agree that there are some out there hacking in MP.
Yup, my overall point was that even if there is no "hack" or 3rd party tool/script, etc...there definitely seems to be some sort of exploit going on. There's too many instances of this happening now to just be a simple server glitch happening on its own.
