When he gets close, he leans over you, then with the scalpel selected (cursor changes to scalpel) click on him.
Yeah, not ot offend anyone but it´s kind of astonishing how many people disregard adventure games when they get stuck when that´s kind of part of the gameplay. An adventure game you never get stuck it´s like a shooter where the enemies kill themselves.
I do get it though, as I get old I´ve also become less patient. But never felt this game was difficult by any means, not many thing to click on, a small map... even in that situation, you have a pointy object and a hostile creature ignoring your pleas that circles around and from time to time gets close enough... I´m no genious and it was pretty obvious where things were heading once you got the object.
