The game does not cheat. It is based on pure random number generation, slightly weighted by the luck stat but with negligible difference (I have had 20-30 luck and didn't really notice a difference as compared to something like 75 luck). 50 Luck (the starting amount) is purely balanced, with lower luck weighting the rolls slightly in your disfavor and vise-versa.

The amount of successful rolls defines the attack power and accuracy.
If the attack has an additional status effect, it requires all rolls to be successful to trigger.
Hence if an attack only requires one roll (like a dagger for example), the effect will always trigger with a successful attack.
Use focus to guarantee successful rolls and increase your chance of triggering the effect.
There are enemies with a dodge icon by their HP bar. These can only be hit by perfect rolls and will dodge everything else, like even 5 successful rolls out of 6.
There are abilities and statuses that provide guaranteed successful rolls for yourself or guaranteed failed rolls for the enemy (like shock). Works both ways.
There are weapons that break (indicated by a crumbling hex icon) that will simply disappear upon a critical miss (all failed rolls).
The chance of success for a single roll is defined by the stat the weapon uses. So if you have 85 STR, the default roll chance for a standard attack of a STR-based weapon will be 85%. Extra abilities on weapons usually require more rolls and/or have a smaller chance of success. It's pure risk/reward and Focus management.
Finally the 'enemies are have better rolls than me' is a psychological thing. There was a guy here a while ago who claimed the rolls are rigged and did an extensive testing only to find out that it's all fair and square statistically.
Maybe take a mage into your party because his Refocus ability will really help you mitigate lower chance rolls due to restoring Focus often.

High evasion helps mitigate damage a lot (with higher speed characters having a higher base evasion), as it can completely negate an attack.
Death Mark can be a tricky thing to deal with. The good news is that you have a decent number of turns before death occurs (not sure what the actual number is right off), but you need to defeat all the enemies first. If nothing else, the death marked player can flee after about a turn if you are not in a dungeon, making the fight harder for the other characters but saving the one.
speaking of which, it's a good idea to distribute armor, evasion and resistance between your 3 characters, making each focus on one. It maximizes the effect of each
