There are other signs though. Some parts will start to make noises before they die. Blue exhaust is another one I think. In other cases, the engine can continue to run with a broken part for a short time, and you'll notice that something has gone wrong through the temperature, battery, water, and oil gauges.
Part conditions are randomized the first time you play the current version (new game or existing), and most of your parts will be between 20-30%, though you'll have one or two that could be a lot closer to breaking, so your engine might sound 'off' right from the first time you play this version.
