Hard difficulty!

So I have been trying to get the Hard difficulty achievements, and for some reason they won't pop up even if I've beaten many different levels on the hardest difficulty. And next to the missions there are 3 medal-things you can get, but there is no logic behind how to get all 3 of them. Because sometimes I play on easy and get all 3 of them, then if I play on hardest I get only 1. It would be helptful if there was a text describing what they mean and some consistensy of how to get them.

One for mission accomplished, the middle for not losing tallarn units and the upper one for not losing Imperial units...i suppose...
