In all seriousness, though? It's probably a lazy holdover from the older games, back when create-an-entrance wasn't as in-depth as it is now.
There would obviously need to be different motions available for title entrances, that way you could have The Rock raise the belt to the crowd on the turnbuckles, have Roman Reigns lazily drape the belt over his shoulder like a douche so that nobody can see the front of it, and other entrance motions that require a belt in-hand to make any sense, and likewise some motions might need to be restricted to avoid clipping,(Some moves where the wrestler crouches down, for instance, would look really weird with a title belt around the character's waist.) but with a bit of effort I have no doubt that 2K could find a way to let players customize their title entrances.
The "improvements" being made every year by 2K are honestly pretty terrible compared to the stuff Yukes was adding back when they were the only ones working on the games. 2K still hasn't brought back Create-A-Finisher or Create-A-Story. :\
