Pretty sure their birth-name is unknown.
We don't know, and presumably it's not for us to know.

Pretty sure they is used for gender ambiguous like flak from BL3, madelaine is a "she".

I'm not using the incorrect term.
You can still refer to someone as "they" or "their", regardless of their gender.
Like, for example; (a boy) 'Their computer is up to date!'. You don't always have to define their gender, when speaking of them.

Actually you do, that's part of the respecting the pronoun people want themselves identified with. They is third person plural, the exception is for when it's referring to people who consider themselves gender ambiguous.

The devs are probably not going to reveal this info as it's rude to tell people a trans persons dead name, fictional or not. (Also for the other comments, singular they has been used for centuries and is not just for referring to NB people)

They in the past was used to refer to people when you didn't know their gender. For example the criminal attacked last night, they ran away before the police could arrive.

If you want to understand all the usages of they here's a nice link explaining how to use they in the singular:

No. Alex is the name of Theo's sister.

If the devs were already feeling like they didn't want to out Madeline as trans, which is understandable by the way, I highly doubt they'll reveal her deadname. Someone's deadname isn't any of your business. (But I do get that you asked this question out of genuine curiosity and I don't want to be rude, it's just an explanation)
