So, I've collected all the hearts in area 1, but what is that greyed-out rectangle?

It's a casette. If you can get it, you'll unlock the B-side for that chapter.

That is a Cassette tape. Every chapter has one hidden somewhere. Collect them to unlock that chapter's B-Side level. These are basically a hardmode level following the theme of that chapter.

The collectibles are more or less shown in the order you encounter them, so look around where the strawberries before the tape are.

They are moderately hard to find, usually behind fake walls / breakable walls / other tricky things. Not as hard to find as the blue hearts though. You'll know when you found one of their rooms when the soundtrack becomes more 8-bit synthy and you encounter alternating blinking blocks. The one that's easiest to find is probably in level 2.
