I'm on Chapter 3 but couldn't find that one either, I think it's related to the colored birds but can't really find anything to do with them

Select the Chasm section, go up 2 screens, you'll end up here: https://imgur.com/a/g8INj
Hold on the machine to make it move and quickly enter in the space on the left, the tape is there.

I didn't find Chapter 1's heart yet, looks like it's related to the colored birds and the antenna.

What's chapter 2's heart? I know you have to get there via dream because of the poem and I know it's just out of frame, but where?

You have to use the 8 directional dash at the right place to get the first chapters heart.
