2 Questions
1. For the "Real Gamer" Achievement, is collecting all the strawberries necessary?
2. Madeline's Journel has a "Full Clear" section. I've gotten all collectibles in the first 3 chapters but that area remains blank. Do you have to collect everything in one playthrough?

A "full clear" star/timer is recorded if you collect all red berries, the heart, and the tape for an A side in a single run. You cannot return to map and use the checkpoints to save time and still have it recorded as a full clear.

HOWEVER, is there any penalty for not "Full Clearing" a level? Does it effect the ending?
Do I miss out on any achievement?

No you don't miss on anything, just a star replaces the flag of the levels on the journal
