When it comes to story mafia 2 does a way better job IMO, you actually care about more characters than just one here(Unlike just Roman in GTA IV) and the missions are more innovative and not very repetitive.
Mafia 2 has better graphics unless you use the iCE enhancer for GTA IV. The gunplay is better in mafia 2, but has more restrictions and doesn't let you do things such as drive boats, fly helicopters, and parachute. Mafia 2 is a bit more realistic(which is both good and bad).
GTA IV has a strong multiplayer component and a lot of mods while Mafia 2 has no multiplayer, and not very many interesting mods at all.
In mafia 2 all you really got is a really good 12 hour story, but there's not much to do in the world besides that, but both games will still get your moneys worth.
A good game, but very different, as already pointed. If you really want a GTA substitute, you should really consider Saints Row: The Third. This game is really similar to GTA IV (arcade-ish gameplay, freeeee roaming, even the map has 3 main islands...) Saints Row seems vulgar, but the acting (specialy female 2 with it's western european acent - probably a tribute to Nico Belic) make the characters more noble, likable and real than most sociopathic good boys in games like Far Cry 3 (terrible) and Driver.
