I assume you already know that the optics are specific to classes though. Even though the whole point of picatinny rails is to be universally applicable, the developers sort of ignore that concept and make things like the 2x holo only fit rifleman, some support, and some marksman classes. Things like the AK74 or even more baffling, the MK18 in specialist class won't take it. Those sorts of rules apply to all optics.
I've heard of people saying what you speak of though. Even though I haven't seen that myself, I don't doubt it happens to some. Just remember when you are choosing your reward, if you hover over the reward it will tell you below in red or green text whether or not it will fit a weapon in your inventory.
If you're on a tether or corded PC, I'd try to screen record and catch it in the act. Most systems have the ability to screen record with no special software these days. I don't know how or if you can with a quest through.
