Level 44 Three Stars

I found myself can't get three stars in this level.
Because I can't get extra heart dealing patients with red or blue pen.
I drag the scroll but there is no extra heart so the patient can't get gold hearts.
I hope someone can help me with this.
I have seen the video on Youtube https://www.youtube.com/watch?v=gw6af3lUfNI
He ticks the patient with the red or blue pen and he can get the gold hearts.
Is there some bugs?

But that website can't solve my problem.

Look at the young man I take care of, he has three hearts. and need a red pen to drug.

After I drug it,he has four hearts.And he wants to go on another treatment

The other treatment is clicking on the strange area in his head X-photo,I got three hearts,and AN EXTRA rewarding heart,so he now has 4+3+1=8 hearts.

So,I Doubt that the ticking activity doesn't get extra hearts!

But I have seen the video on Youtube.He can get extra hearts in ticking activity.

So,I asked this question.

The red hearts broke the golden hearts combo,that's the reason why I can't get muck grades.
