Do you have a save you can reload, from before the cultists showed up? That could be an easy solution.
Does using TCL disable achievements? I think you're right that it's the second cultist that is lost in the terrain because there is only one laying in the street currently.
But as Rez states you don't need the note, you can talk to the captain of the ship at any time..... I wouldn't before I was level 30 or higher, but you can....
