You still don't get to choose which effect it will have. The game will choose the most valuable effect to determine if it is a potion or poison, then removes the opposite effect.
You also don't get to choose whether or not to use the perk, so if you are using alchemy to make money you may end up with less valuable potions to selling, because they are missing some of the effects they would normally have.
If ya looking to create a stamina drain, then Scathcraw is what u want.
Being able to make a potion worth far more than any merchant ever has, even an upgraded Thieves Guild fence with thousands of gold, is kind of broken. You don't even need any perks for it either. (Though that is the fault of a few ingredients, not the alchemy system as a whole.)
