To fast travel via the world map, open the map and click on the icon of the location you want to go. Only locations which you have been to or “discovered” can be traveled to. On the map, the icons of these areas will be a solid grey color, locations which have been revealed on the map but not actually discovered will have black icons and cannot be fast travelled to.
As a side note: this way of fast traveling will not work if you are in combat, if there are enemies too close to you, if you are indoors, or if you are over-encumbered (are above your max carry weight).
Carriages can be found near the stables of Whiterun, Solitude, Riften, Markarth, and Windhelm.
Talk to the carriage driver and ask for him to take you someplace and you’ll be shown a list of destinations and how much it will cost to be taken there.
All Hold capitals should be options. I can’t remember if all the villages are too, or only some of them.
You can utilize the carriage even if you are over-encumbered or have not discovered the destination beforehand.
Ferries can only be found at the docks of Solitude, Dawnstar, and Windhelm, and they can only take you to the docks of these three cities.
Like carriages you will have to pay a fee, can use them if over-encumbered, and can travel to any of the three locations even if you have not discovered them beforehand.
There is one method of fast travel that works like this in survival mode, but it does require advancing the Dragonborn DLC far enough to get it. Though it does have its own limitation, and can't travel between worldspaces (so can't go from the mainland to Solstheim, for example). The Bend Will shout allows you to fast travel while riding dragons, even in survival mode.
