I'm an old git too, nor am I stupid, but your starting position is so confusing and contradictory I have no idea where to begin to help, except maybe you should ensure your starting tattoo colour is black or dark red as sometimes it defaults to the skin colour under some skin colour mods...
No. I'm saying I've done nothing to make the tattoos stop working. Even though they show up on male characters, they've stopped on female ones.
Let me explain.
I'm in racemenu. I've created my female. Now I just need to add a delicate shade of tattoo to her body.
I've been caught out before by not seeing any tattoos. Then realising that I've not coloured it. So I always make them plain black to show up, and once I have the tattoo I desire, then I change the colour. It all worked perfectly for my Orc yesterday and Nord a couple days ago. But they were both male. But they're just not showing up for the female.
This I cannot explain.
Just quickly popped into the game, and I can get the tattoos showing up. With or without clothing as I know sometimes, tattoos vanish when you remove clothing.
So I haven't a clue why they were not showing up earlier.

Were you using an Alternate Start mod (Depending on which one, I would make the case Lighting was Hinky) or were you at Helgen with Vanilla Start (This is where the Overlay not applying would make most sense)?
Post your load order, and your specs, so people can eliminate a bad load order.
I do use the alternative start. But it's the first time it's ever happened in "?" of years. Just weird it mended itself. Though I used the consul command "showracemenu" as I had saved then re-logged back in.

Sometimes stuff gets weird. Last week on my previous playthrough I noticed two candle flames under my Character after exiting the starter cell, They were annoying me for 30 levels or so just there. Then one day they were just gone. No explanation.
With a previous character, after restarting the game, I knew that as soon as my character got hit, my game would CTD. So as soon as I restarted the game after shutting down the previous session, I'd punch the first person I saw, get hit, CTD, restart and everything thing would be OK Only ever seemed to happen to that Nord.
