How do you modify the difficult in the ini?
Sky UI got bugged with the more recent updates due to Bethesda modifying the UI. That mod patches it so it works properly again.
It didn't work =(
Note: The INI setting related to game difficulty is the same as the setting in the game menu. If the INI is set to 5, the game menu should say 'Legendary.'
With the game difficulty persistence fix mod installed, you may try creating a new character and see if the difficulty remains set, as well as whether you can toggle survival mode. This way you know if it is a game issue or a save file issue.
If you are using the current game version, you can delete the INI files and use the Steam game launcher to rebuild them.
You may also try BethINI PIE to make several adjustments to the files.

Also, with the current version of the game, you can try a file verification.
From the Steam library, right-click Skyrim SE > Properties > Installed Files > select 'Verify Integrity of Game Files.'
