Some spells won't trigger maximum Destruction if they don't have the right keywords. So, say a spell from Spectraverse won't trigger the gore as an example.
Still double check the requirements. This is the most common cause of Meximum Carnage not working, it's requirement MUST match your version of Skyrim or it won't work.
If that's not working are you using MCO without the Maximum Carnage patch?
Or are you mix and matching Maximum Carnage's newest version with stuff like the subsidiary mod that makes gore happen on every attack, which only works with an older version?

You have to use the Maximum Carnage patch for MCO, it's included in the Maximum Carnage installer. Make sure you have the newest Maximum Carnage version installed from Nexus as that's the one with the patch you need!
You're definitely doing something wrong then, it works. Re-download the dependencies, make sure they're for the correct version of your Skyrim, Maximum Carnage is version agnostic but it's dependencies are not.
Then go in-game and use a kill move or power attack on an enemy. Preferably a human one.
If that works then you know your dependencies are correct and you can test out maximum destruction on human npcs. Fire magic is best for this. Use vanilla spells. Not all mods will be supported, they will need the proper keywords.
Don't rely on Vortex to tell you if it's correct or not. You need to manually check the prerequisites to make sure they're the correct version for your Skyrim. I've made this exact mistake before, it's literally a problem with prerequisites or you're using a modded spell that doesn't have the correct keyword to trigger.
If you're 1000% sure that you're using the right prerequisites by triple checking and re-installing the prerequisites then you may be having issues due to extreme script lag (unlikely but possible) or the newest version could be bugged (also unlikely but possible, works for me on 1.5.97).
