If it was a fair death, I just play a game of Kohan 2 Kings of war to fight to get my soul back and reload.
What you can do is to have a rule when starting a new game: if you die, then it's game over. You have to go to the main menu and delete your game saves. Granted, this require some discipline but I would advise to go this route because it happens that you die by something that is not totally under your control, such as trap rocks that got touched by your follower and it was just that hard enough (ping pong effect with the surface) to kill you if it hits your character.

That and Dragons and Bandit Mauraders Triggering a Kill scene when you have 90% health. Yeah I got Violens and shut that stuff off for NPCs, Thats just BS (But of course if I do it, its ok. WINK, WINK)
however, if you often lose so much progress after dying (like several hours worth) it's a worse punishment trying to re-accomplish everything you did than it is to just delete that folder alltogether and start anew.
There can be Permadeath in this game.
Step 1: You buy the game.
Step 2: You start playing.
Congratulations, Permadeath Man! You're in Skyrim.
If i die because of my own actions, IE, i stood on a mine in Fallout, or a trap in Skyrim, then that overrides those 3 and i restart right there.
I keep my saves though, until i run out of lives, because it's a Beth game and Bugs can and very much do happen. If you delete them all, you have no points to go back to if a quest is bugged out, and restarting the game because "X" quest doesn't work is a big kick in the teeth....
