Edit: To clarify it is a Vanilla Game issue. Pathfinding AI is very finicky. It can very well be caused by script lag but its not really solvable by less mods, it will happen at some point regardless even on Vanilla Games.
Edit2: I mean, chances are just restarting the game (save and Exit, then restart and load save) will MAYBE solve the issue if it's what I think.
I guess the only option is resting inside a cell and seeing if he resets. I would do it from a save before coming to Riverwood no closer then embershard (even if Ralof goes ahead, the scene should start playing once you reach the walls).
Thats the only thing I can suggest at this point.
use the resetAI command
Another option is to target him in the console, and use the disable command followed by the enable command.
This does essentially the same thing as the resetAI command but sometimes works when resetAI does not.
