That or use a mod to force-equip apparel/weapons and save them to the outfit.
I tried this gave her like 6 different sets and she doesn't want to equip them, only the helmet and weapon.
Her base model I think doesn't have a helmet
There are Mods and Console Commands to get around that however. I use AFT which has an outfit manager, lets you customize what they equip for both armor and weapons.
Nether's Follower Framework is one but as the name suggests it's a complete follower framework mod
