So according to the UESP (the go to for all things Elder Scrolls) they mention a possible fix for this quest not starting.
"The quest may fail to start after completing the previous quest. If this happens, enter the following command into the console: setstage dlc1vq03hunter 5 then wait a few hours for Serana to appear in the fort, then follow Isran.
