As I've just concluded in my thread Skyrim survival is a very badly done waste of time. I've developed a character to level 45 in the unmodded version, and I feel it has been a complete waste of time and money.
As it is, the three survival needs of food, sleep, and heat are nourished in totally different ways and each tied to a different core stat. Introducing thirst wouldn't do anything new really and it would clash with the existing symmetry of it.
It's fine that's how you wish to immerse yourself, but real talk, it's not a survival mechanic if it's just a "slight disadvantage". What exactly are you surviving then? Just call it what it is, an immersion addition, not a survival addition.
Even at the worst you could still function without much of a penalty after being hungry because the only penalty was your stamina. So you cna not power attack, block, or sprint whoop te do that got nothing to do with a sneak attack out of stealth or a fireball or better yet kill the mob quickly in melee and you never have to power attack or block more than once.
