Just killed the reaper without getting any of the gems

I just entered and then he was already there with the minions. Is this a bug or some thing?

According to the wikis there are other possible ways (certain shouts, spells, and console commands) to trigger an attack without the reaper gem fragments. So while it is rare it is not unheard of. Consult the wikis if you are interested in details.

I just entered while casting the muffle spell and the minions were ALREADY awake, and the reaper appeared shortly after that

Ah. Cool. Think I have only made it to the Soul Cairn a few times (Maybe 3 in all). I vaguely remember them but not anything about them. Can't do Dawngaurd, Won't do Dawnguard. It's shut off in my games.

Not exactly sure how you would get to the Soul Cairn if you won't do Dawnguard.
