HOWEVER if you wish to still pursue this, there is a new but stability-not-confirmed method.
Using Project Proteus mod. Where you can save your character appearance and load it unto any NPC you desire. Though at a cost, it cannot be custom race. Vanilla race to vanilla race will work. Same custom race will work. Custom to vanilla will not work.

The best proven method is to learn how to make an NPC replacer. This is not easy and you're left to work with a lot of external programs instead of being in the game. Little to no script that would make your game go bonkers. Though sadly cannot do it on the fly.

BUT using alot of the features of project proteus is basically abusing the console. there are alot of features that you can use that might cause problems down the road. since i did a clean install and started using vortex i have not broken my skyrim game. i have broken some saves trying to get the load order i want, as uninstalling mods that use scripts during midplay is a no-no according to the elder wisdom of others here. i listen to what they say, since when i was modding on my own with no thought given to the "right way" i broke my game out of ignorance.
this is just my piece on it. i am far from an expert, and i won't say "go ahead and try proteus" cuz as KNDY said above there are some real concerns with this mod. If you are modding "the right way" using a mod manager and your mods are in staging folders and not actually modifying your skyrim install, and you are ok with breaking a save and having to start over with a new playthrough, then it's a fun way to play the game.

Northbourne has NPC mods for just about all of Skyrim.
This mod will update Lydia as well as Ysolda and everyone else in Whiterun you can think of.
Not sure if this is what you are looking for or if you are looking to go totally custom for you.
Northbourne mods are best with the secondary mods installed such as Biijin Skin, Skysight Skins, Superior Lore Friendly Hair, Beards, Brows etc... The list is on that page I linked to.
Installing Northbourne for all of Skyrim means you are installing about eleven mods, but you are replacing almost everyone in Skyrim at that point and this is a TON faster than building your own individual faces.
