Probably the best course of action if possible. Console commands can be dangerous af if you're altering important stuff like that.

You tried teleporting him to you right? I feel like maybe that could have made the system funk up and glitch out.

Open console with `
Left click on them once (Your spouse)
Exit Console
Go menu and then load in your main/latest save
Once loaded, open up console
Type "moveto player"
Just me and 'Prisoner', which is the starter. I don't know what's going on here :(
I've never actively added mods, but I am using the Special Edition, not sure if some mods are automatically included?

to give non essential npcs chance for survival is to give them some ultimate healing potions. or you can do console command like: setessential 1c194 1 or mods.
