The The UESPWiki says:
Disarm is a shout that pulls the weapon out of an opponent's grasp.
It is one of the more common shouts used by draugr, which can be quite an issue if you are wielding a unique weapon, as the weapon can easily be lost when thrown from your hand.
Normally the physics engine detects when objects collide, and causes them to bounce apart. Sometimes it fails (especially with fast-moving objects) and they go through each other. This may cause weapons flying from your hands to fall through the bottom of the world.
It's more likely to happen if your frame rate goes above 60FPS, as exceeding that figure causes the physics engine to misbehave in increasingly bizarre ways. Send it high enough, and eventually it'll decide that your character needs to swim everywhere.

find the name of you weapon and get the REFID
player.moveto REFID
tcl (so you can get out of the floor)
for me this was 20 feet underwater beneath the stone floor :)
I find that if you don't learn the disarm shout then Deathlords don't use it on you, but others disagree. If a decent weapon is lost, you have two choices, reload from before the fight or live with it.
Depends, the one where the jagged crown is will use it even if you never started the dragon rises quest or visited any word walls.

also sometimes the graphics for your weapon get disabled, you can still see a floating text tag though and pick it up that way.
The Centerpoint of the cell (The space) or a Zone in point. Check up or down. Items in the void have a tendancy to fall perpetually or continuously. You just have to keep looking while TCL in console. Sometimes if it's an option just reload the save is easier.
Check corners and crannies in the area where it was disarmed, etc. Game Physics are weird. Also check ccompanions to see if they picked it up (Maybe NPCs also).
