Second issue is there are a LOT of NPCs in the game. Even if you only did male or female NPCs, that's a ton of commands you'd have to put in it because each one would need a separate line for them to remove their footwear. Maybe some programming guru could write a simple script or something to do this, but... that's a lot of lines if it's not possible to be scripted.
The other thing is, on a cell reset, you most likely would have them rewearing their footwear, if not earlier. NPCs like to wear their "best" gear depending on where their skills are focused. If they have footwear in their inventory, they'd probably keep rewearing it on their own. Not sure if that's true though, never really looked into it other than sets of armor for my followers.
About the only way to be really sure that noone would have footwear on (or whatever limited set of NPCs you wanted to see with bare feet) is to go into the CK and manually remove the footwear they have in their inventory then save it as a mod. You'd also have to go over the leveled lists for random NPCs like Bandits and such as well (if you wanted them to not have footwear). Not sure how far you want to push this because that is an enormous amount of work.
