Depends on the circumstances leading up to her disappearance. She is probably dead.
I didnt think of that, but i assumed she wasnt dead or im sure the OP would have mentioned it, atleast common sense would have.
You would think, but personal experience would tell me otherwise. When I first started playing on console a couple days after the original was released I lost Lydia to the cold embrace of death. Didn't notice for a little while either.

No, that is true. Enemies cannot kill your followers.
Yes, they can. The only reason they typically do not die is because the AI is programmed to stop attacking them once they get incapacitated BUT if they hit them again while they are kneeling then they WILL die. I watched Lydia get pulverized by a giant more than once.
Again, you're wrong. I can get into game real quick and you can spectate me. I will do what I did when I tested it years ago. You will watch her die while I stand by and do nothing. A quick search online would prove you wrong as well.
