I use only Immersive Citizens and my NPCs most of the times just run away and almost* never get themselves killed.
*Aerin and Vulwulf Snow-Shod got killed once when fighting vampires in Riften. And Nazeem with his family were killed outside of Whiterun gates during civil war quest. They probably had lived if I had "Open Cities" mod. But who likes Nazeem and Aerin anyways?

The NPC will come back to Life but he will not do his Job anymore.
He will not visit his Store.
If you talk to him then you see no Sell option.
one way to 100% to prevent those attacks is befor you enter town quicksave and save
try enter town if vampire attack happens reload wait round 24-48 h reenter town this mostly prevent those attack from happening to begin with
can do this on new game play threw that way only scripted npc die when there death is preset wait with bleakfalls barror untill you ready with dragons and vampires
that way you can do misc quest in all holds
befor you start up bleakfalls barrow the vampires and dragons and the vampire spawns will be less and you will have 0 dragons.
vampire attack happen due to weather, certain days and how long it has been that you killed a vampire group
if you hear the reset tune bandits,vampire attack and all these will be respawn this tune you hear once a week and it reminds you that cleared placs have rerset as well.
1) Yes, mods disable achievements. Thank console companies for that. It is a known issue.
2) Bethesda won't fix anything in the Special Edition that had not been fixed in original Skyrim.
3) Use the mod enabling achievements in a modded game. Or do not fast travel to cities. Always fast travel to the stables or any other location very close to the city and enter on foot. If they do attack, they should be facing you directly, not running amok down the streets.
