Report a translation problem
It's a vanilla bug. Have not yet found a fix for it because I'm not quite sure what causes it.
Open the mod and see if it's a leveled weapon.
Even if the mods themselves are good, an incorrect order can produce some weird effects.
If you are using mods that affect followers, you can not then say it's a vanilla bug, because the mod may be affecting them in this manner.
It may be because of levelled weapons, but dismissing it as a vanilla bug will be getting in the way of others finding a solution.
There is no script or mechanic that does such things.
I have made several followers in mods and always use levelled gear on them, Never encountered duplicate weapons.
Its a bug from something, Not a feature.
To clarify; the bug doesn't duplicate the sword in my follower's inventory. There's still only one sword listed.
I caught that part. But not familiar with the bug.
Can you post a mod list?
Also, Highlight said npc in the console and use " showinventory ". Which will display items not visible from the normal inventory container. Like default outfits etc. Still only one copy of the weapon?

Its not sorted properly, But possibly because the out of order amiddenborn plugins are for oldrim... despite what people claim, Form 43 plugins on sse cause problems. Unless you ported it yourself?
Unlimited amulets sse has bug reports of jewelry stacking repeatedly, Not sure it that could affect weapon slots. Last update was 2016.
Visable favorited gear probably affects weapon slots. Bug reports are disabled.
I would definitely start with the mods though.
Unlimited Rings and Amulets seems to change their equipment slots to the same as the Bandolier mod uses. They use the same shield icon in the inventory screen, anyway, so I'm very much doubtful that that's interfering with weapons.
And what is "form 43?"
I am more surprised you arent getting form 43 warnings in loot. aMiddenborn has plugins. Ive ported it myself and know that mod inside and out. It wouldn't be in your load order without a plugin.
It modifies equipment slots so you can use more rings, weapons use equipment slots. Ive tried this before as well.

While it might not be your issue, It's certainly modifying similar parts of the game. Not to mention the bug reports for stacking jewelry. But maybe not.
The format of oldrim plugins. 44 is special edition. Even bethesda states 43 in special edition causes problems. In the case of aMiddenborn it needs to be re-saved in the CK, Meshes need optimization, And you can convert textures to bcn compression for better performance. Or you can hack the mod up and oly use its direct texture replacers, No .esp, No meshes. But neither is the case here.

View the File Header to see the Form Version. A Form Version of 44 means the plugin is already converted for SSE. A form version of 43 means it has not been converted for SSE (PC can use either form version but consoles require 44)

SSE supports all form versions up to 44, older ones DO NOT corrupt saves. Badly made mods and conflicts do but this is not related to SSE in any way and form versions particulary. Don't spread the false myth please picked up from a random youtube video from someone who is not even a mod maker himself afaik.

"Except that has proven to be a rumor. I ran several tests for many hours and have yet to experience or hear any reports of a corrupted save. The form version in the header is only an indicator of what software saved the file, and has no impact on your game. A plugin with Form Version of 43 in the header by itself will not cause save-game corruption. If you're still not convinced google the issue, you'll find that while everyone suspected file 43s were the cause of corrupted savegames, when they troubleshooted and converted to 44 the issue persisted."
And, personally, in my fully stable CTD free game with a mod load of 1535 total mods, but, more importantly, 1103 plugins merged down to 233 with 338 of the 1103 mods being form 43 and never having been changed to form 44, there have been NO problems whatsoever after 125 total hours.
Finally, once a false belief becomes accepted as a fact, it has a life of its own, and is nearly impossible to refute. This has become the case with the Form 43 issue. There MAY be ONE particular record which if used and not changed to Form 44 MAY result in problems, but it is exceedingly rare - if indeed it does cause the problems some have claimed for it. Those who believe Form 43 causes issues will not be otherwise convinced regardless of what the facts truly are and regardless of where the information is from..
