It is a detrimental effect. Getting thirsty in Sacrosanct is a bad thing. Or at least intended to make you feel like there's a significant effect. It's a trade off with:
In summary: Thirst reduces your endurance and has other negative physical effects, but thirst also changes your magical abilities into [arguably] more powerful ones.
I learned all that just by reading. How fun.
As for the other negative effects, i'm already aware of them, and they actually do make sense.
The point is, it's a trade off. It's so Vampires don't just have big bonuses. That right there is plenty of a point to it. It also makes you want to end combat ASAP and get the kill instead of running around spamming a heal while getting peppered with arrows, for example. It's not hard to think of game play and role play reasons for it to be in the game.
And I dunno what Skyrim you play, but being at 25% health doesn't mean I'm "likely dead"; it means I still have 25% health, all of my pots, and whatever magicka I have left too.
