1) Avoid taking damage. Get good at blocking and movement to avoid hits. Get high armor and resistances. Play smart with shouts such as Slow Time, Become Ethereal, Whirlwind Sprint and Unrelenting Force.
2) Use alchemy and stock up on potions.
3) Get a huge health pool to survive combat and rely on out-of-combat health regeneration. Go grab The Lady mundus stone for extra health regen. Find a ring and necklace with more, at lv 16+. Radiant Raiment is a good place to look. (Might want to prioritize resistances before regen, though)
4) Outdamage your enemies.
5) Become a werewolf, and against human enemies rely on feeding to heal yourself.
6) Get a weapon with the Absorb Health enchantment. Buy from a smith, find, or grab a unique like the Nightingale Blade (probably out of the question since no thievery?) or the Ebony Blade.
Only place I can see where you'll have issues is against Dragons. Depending on the difficulty level, you may find their breath attacks OP. If you are allowing resistance enchants, then it's again just a normal playthrough. Master/Legendary without those resists are going to be brutal against Dragons. Might have a few problems against Dragon Priests as well if you aren't allowing them.
If you want a solid challenge of this sort, throw in one of the Bartering overhauls that reduces sell prices and makes the economy not so "I have 1 million Septims at level 10"ish. Can actually be a huge gamechanging, especially if you restrict selling items to only gemstones, potions and scrolls. The economy in Skyrim is one of the worst I've seen in an RPG. Really unbalances the game with how much you can amass in a short period of time.
Concentrate more on evasion, blocking, and attacks of opportunity and you will do just fine even on the higher levels of difficulty.
