Can't start Lost to the Ages quest
I read the book but it didn't give me the quest. Misc or anyother quest. I went to the ruins and the journal wasn't on her body. I have never touched a shard or anything I don't have mods installed and I just want the achievement. Any help?
EDIT: I also finished the dawnguard questline as a Vampire and killed the Leader.

if you read the book at a lower level, it will give you the misc quest "investigate the ruins of arkhingthraz" or something like that. at higher levels it SHOULD give you the legit quest. try reading an untouched book, if you can find another one.

>make sure you go the the right cave. (sounds like you did though)
>you may have to use console commands to reset or advance the quest stage.
> look around in the cave for her "ghost." this should continue the quest as normal.

Did you get Katria's warning speech when you entered the Ruins? If not, she might not have spawned. A couple possible fixes would be to exit/re-enter the Ruin, and also to save inside the ruin (on a fresh save of course) then quit and reload that save. That might force her to appear.

She also might be stuck somewhere in that first portion of the Ruins. If none of this works though, you are probably going to have to reload a save where the quest itself isn't broken.

Not sure if the UNofficial Patch fixes any of this behavior though, you might look into installing that at the minimum. If you want Achievements as well, you'll need the Achievement Mods Enabler. Two mods though and you will have a much smoother, less broken questy, playthrough.
Check the Wiki, I think if you've completed a certain quest then lost to the ages doesn't start.

This quest may not start if, after installing Dawnguard, you have visited Mzulft or Deep Folk Crossing prior to reading The Aetherium Wars and the corpses at these locations have been fully removed from the game.
PC Only This bug is fixed by version 2.0.1 of the Unofficial Dawnguard Patch.

This what you mean?
