You probably shot her.

The only way Lydia can die is if you kill her.

Yup, we all die sometime.

Yes, she is dead. You can supposedly use console commands on her corpse the resurrect, but you will lock out achievements. You are supposed to be able to open the console commands by using the ~key. When I try it my computer just beeps at me. I think it's because I use a controller instead of the key board.

use quick save before you entered cave and you can watch her die again.

Actually, you will only lock out achievements because using resurrect command on unique/named NPCs will most likely break your game. It is something that you should not do, as Skyrim runs a death script for these NPCs that is baked into the save - and this cannot be undone. As far as the game is concerned, that NPC is dead.

Not quite. I've seen her die to mammoths with no player interference. Hell, once she was just puttering around Breezehome and got sent to the dead npc cell for no reason.

Lydia is 'protected', which means she can technically only die by player interference.

Console commands do not block achievements. Mods without an enabler, and only mods without an enabler, lock achievements.

If she died you can just use the console to resurrect her. Using console commands does not turn off achievements.

If you are near Lydia's corpse:

1. Open the console.
2. Click on her body to set it as the target (you'll see "000A2C94" appear at the top middle of the console if you you've done this correctly).
3. Enter the console command: "resurrect"

If you are not near her corpse:

1. Open the console.
2. Target her using the "prid" command: "prid 000A2C94"
3. Enter the console command: "resurrect"

She'll now be alive, however, when an NPC dies the game flags them for dead body clean-up. Depending on how long ago she died, she'll either be there arlready, or she'll be teleported there at some point in the future (even if she's now alive again, the command to teleport her in future is already set in place and will still execute). So she'll be stuck in the dead body clean-up cell. You just need to move her back to the game world and then she'll be back to normal, she'll be alive and not get cleaned-up again, unless you let her die again.

You can teleport yourself to the dead body cleanup cell using this command: "coc WIDeadBodyCleanupCell"

Or, you can just use the command to teleport to her: "player.moveto 000A2C94"

When you've confirmed that she's in the dead body clean-up cell, you can either load a save before you teleported yourself in there and use these two commands to move her to you:

"prid 000A2C94"
"moveto player"

Or, like before, if you're in the dead body cell with her, open the console click her to target her, then teleport yourself out (e.g. "coc whiterun"), then do the command "moveto player". Or, you can open the console, click her to target her and then load your save before you teleported yourself to the dead body cell and do the "moveto player" command (The state of the console is not tied to your save, so targeting her, then loading won't clear the console target).

If you need to do this for other NPCs you can find the NPCs' IDs on http://elderscrolls.wikia.com/wiki/Skyrim or http://en.uesp.net/wiki/Skyrim:Skyrim. In the case of all commands above, you want the Ref ID, not the Base ID.

If you want to help prevent certain NPCs from dying look-up the "setessential" command, note that you need to use the NPC's Base ID for this and should not have them targeted in the console when you execute the command or it bugs out and doesn't apply (to clear your console target, just have the console open and click in the game world above it until the "xxxxxxxx" ID text at the top middle of the console disappears - disappears, not changes to another reference - the first time you click something it targets it, if you click it again while it is already targeted, it will de-target it instead, so if you keep clicking in different places you'll never be able to clear your target). Even when set to essential there is a small chance that the NPC can still die, like if they take a massive hit or two or more hits in a very short window, but it is very unlikely.

From UESP: "When your follower would die due to an enemy attack or a trap, they will be incapacitated for some time and begin to recover their health, just like essential NPCs; enemies will stop attacking or targeting them during this time. However, if they lose all their health again during this period, they will die for good. Thus, you should try to move the fight away from them, so that they are not hit by a stray arrow or area-effect spell. They can die from a damage-over-time effect that is stronger than their health recovery; most commonly those are caused by poisons."

Yeah, basically if your followers dies in combat... you were the one that dealt the final blow.

If they go down on one knee or have very low health, be very careful with arrows and don't shout or cast destruction magic around them.

I have killed Lydia many times with a stray arrow... although I'm convinced she jumps in front of them.

The "resurrect" command will reset them to the last known state they were in before death. "resurrect 1" resets them completely.

Okay, I'll bite. Where'd you get this idea from?
