Modded Game, some naked NPCs
As the title states, the game itself is modded, using mods like Perfect Legion and the Stormcloak armour overhaul. I've noticed some of the factions NPC's wear clothes, but some do not with just their undergarments, weapon, shield and the cloak. There is also a case where some normal non-faction based NPC's have also nothing on, or just have a hood on and the rest is naked.

Anyone know what mod could potentially cause this?

Conflicts in the leveled lists will cause what you are seeing. In your case using Wrye Bash to create a Bashed Patch, or MatorSmash to create a Smashed Patch are probably your best solutions.

loot is no help with leveled lists,they need to be merged

This can also be caused by the game engine, is how it reacts when you replace appearance items of NPCs.

You need to reset the naked NPCs inventory, https://skyrimcommands.com/command/resetinventory

To avoid this issue in the future, don't replace NPCs armors and clothing mid-game.

I suggest you use LOOT instead of Wryre Bash as well (if you believe it's a conflict issue), unless you know what you doing creating random patches can make it worse.

They weren't replaced mid game, These mods were added with a fresh start, I did use the resetinventory as well but half the time I needed to re-do it twice for it to work.

Then you have a compatibility issue.
Check for compatiblity patches on mod pages?

Hi, found this on Google Search and wanted to add.
This was happening to me, Stormcloak Soldiers were naked at random places I went. Turned out in TES5edit, in Skyrim.esm, some "Outfits" conflicted... some "Leveled Items" conflicted. Stormcloak Outfit conflicted with 4 mods! One of them was in Cloaks of Skyrim, another in a BashedPatch I made, and was a dirty edit. So I redid my bash patch. Just about to test, worst case I'll delete Cloaks of Skyrim.
Watched a Gopher video to understand everything.
