Now, the old version I was using in like 2013 or something did exactly what you seem to want. (For example, spend x number of days in Skyrim, get a bonus to your frost resistance. Kill x number of vampires, get a bonus to disease resistance. It did get quite overpowered IMO.) The new one gives you points for doing stuff which you can redeem for... honestly much more boring looking but probably better balance bonuses.
But I seem to remember the new version had an option to restore how bonuses worked in the old version, so maybe check it out? (I could be misremembering given how long ago that was however,)
And yeah, Ordinator. It's a perk overhaul but it's a very good perk overhaul that adds a lot of interesting abilities to the perks.

adds new perks outside of the perk trees (so compatible to all perk overhauls like ordinator)
with the second mod you can get the perk items as random drops; it's not exactly after X kills like you asked
