Hence, Update included a "warp to player" script that means you never lose them. However, a side effect is that when you're kicked out of Volkihar Castle for refusing Harkon's "gift", the first follower you obtained will warp to the boat ramp, even if you've previously left them somewhere else (i.e. Waiting at Home). Not sure if that happens if you've actually Dismissed them, though. Have to try that.
I always get Jenassa first, since there's no quest involved, and by the time I take Serana home, Jenassa's chilling in my house (UFO "Relax for a while" command). As soon as Harkon gives me the magic boot to the boat ramp, she appears, tells me we're one and the same kind, then proceeds to walk back to Whiterun, so I have to make her Follow me again from that point.
What you're describing seems to be unique to your game and UFO. My first follower this game was Faendal who has remained at Lakeview Manor where he's employed as my steward - didn't even show up for my wedding he's so reliable. I've had several followers meanwhile, the latest being Rayya ( someone has to carry my extra stuff ) who has been outside waiting in all the appropriate locations. None of the interim followers which have since been dismissed have ever shown up anywhere other than where I've left them. It has only ever been my current follower.

Your follower Stenvar is still lost, mate...
PlaceAtMe command creates a COPY of the NPC, follower or object, so what you have now is a clone of your follower. Your original follower is out there somewhere.
The actual console command to move an NPC/follower to you, is the MOVETO comnand.

Thankfully, for such simple needs as finding your follower, you don't need to mess with console commands. What you need is a follower search-n-track mod.
For SE/AE, the following mods also work as an alternative to the old mod "Are You There".
They allow to search-for/tag any named NPC and summon/move-to/trade-with/check-stats/track them (they don't get added as follower). These mods also have a nifty mechanism to create a party and insta-teleport/summon them to player. I use it to find and go to any new mod-added followers, or to teleport directly to a merchant at home.

The following mod does many of these NPC tricks, but also provides other useful (cheaty) mechanisms for ammos, armors, weapons, books, shouts, spells, perks, potions, ingredients, keys, etc.

All 3 mods can be used (haven't noticed any conflicts or issues), and are reliable and working in SE/AE.
For managing followers, there are follower frameworks mods like NFF, EFF, MFF, iAFT, etc., that can surely help.
Get either Vortex or MO2 (Mod Organizer 2) mod management tool from NexusMods.com website, to manage mods.
TLDR: Use Mods to manage followers. and to make your Skyrim experience better and more fun.
