Edit: Also verified the game files' integrity. Still no option to toggle on Survival.

In the main menu you can see the "Creation Club" option where you can download Survival Mode for free this week.
Once done, when loading your game or starting a new one you should get a prompt whether or not you wish to activate survival mode, but you can also activate it via the settings.
I've gotten that far, but I'm in the middle of the wild in my savegame and I don't see survival effects yet, I guess you need to change cells or something to truly activate it.
It might be if you using the early release of SkyUI it removes the survival check box and keeps resetting the difficulty setting to novice.
After you get it installed and load up your game (making sure you let it reload after closing the creation club, if you're on a console, opening once, and then reloading if you're on PC), fast travel anywhere that will force the game to load. I was on a mountaintop and went to whiterun. This get it in gear, and a few seconds after that the menu describing survival mode popped up, and after I enabled it on THAT, the option appeared in my settings menu, before that it wasn't there.
