If you've fast travelled and your horse is still missing... it's probably not your horse anymore. It's probably dead. Eaten by a bear or BBQed by a dragon most likely. You did buy that horse, right? You didn't steal it?
Yeah, that happens. Horses in Skyrim have the life expectancy of a terminally ill housefly. Fortunately you can download mods that remedy this issue in a variety of ways.
use Convenient Horse mod
Well, if you want the full blown experience Convenient Horses gives you a lot of control over your mount. But you have to do a couple of mod-related quests in order to unlock the whole mod. It lets you do things like talk to NPCs and harvest ingredients while mounted. Read all about it on the Nexus.
If, like me, you want something a little more lightweight and less fiddly, I can recommend Warhorse Domino. Domino is a custom horse you can find at the Whiterun stables. He looks different from any other horse in the game so he's easily recognisable (never lose your horse again!), and he's tagged as being essential, so he can't be killed. He's free, he's slightly faster than vanilla horses (but not ludicrously fast), and he deals increased damage in combat, so he'll help you out in those tougher battles. Once again, search for the mod on the Nexus and see if it appeals.
If it's alive, but just mot following, it means you rode another horse and need to ride it again.
