Iron armor/weapons, Leather armor, Hide armor, silver, imperial etc, and many other lowel level armor/weapons pieces, don't have the 2x tempering perk.
That's why iron is only going to flawless, because there is no perk you can get that gives iron a 2x bonus to tempering, like Steel for example. The only way (without mods) to get iron to Legendary status is by using smithing gear or drinking fortify smithing potions.
There is a mod for Oldrim that gives all lower armors and weapons the 2x perk - it was either Weapons and Armor Fixes Remade or Complete Crafting Overhaul Remade, both by Kryptopyr, not sure which one, but that fixes it so they have the 2x perk attached. I don't know if its available for SE yet.

Nothing at all can be tempered, or nothing at all can be tempered past Flawless?
Have you got the perk, because going to 100 in smithing doesn't give you the 2x tempering bonus, you need to get the perk to get the bonus.
Do you use mods?

No, the patch is fine, been using it for the last 5 years. I'm guessing you've either got a mod conflict or you've been abusing console commands.
Edit: correction, sorry. If you have the perk you can improve well below that.
I was at least level 30. But I did "grind" to get blacksmithing up to 100.
I also focused on enchanting skills while out in the field. Leveling up Magic.
1. I got crafting up to level 100.
2. I got enchanting past level 50. (was pushing for 100 & dual enchantments).
3. I enchanted clothing items with blacksmithing skills. (where possible).
4. I used Blacksmith's Elixir.
Instant Legedary even without the Elixir (but not as high in points).
