Skyrim has tons of adult mods, so why was hot coffee mod problem for GTA:SA?

The ESRB rates games on all content regardless of whether it is playable content or not.

Mods ADD content, they do not UNLOCK it.. Simple enough?

So if "someone" distributes the patch that's not inside the game data would it cause problem?

Adult mods in Skyrim just replace textures by moddifed textures. They were never a part of game made by game publishers.
Hot Coffee on other hand was a part of the main game which was cutted so game wont get AO rating.

It's not just textures but tons of modified scripts, meshes and animations. They even 3rd party launcher to extend vanilla scripts.

See because it would call to whatever they have locked inside to activate those scenes.
Meaning it would expose what's hidden in the game, therefore the Hot Coffee loophole happens in real time.
